What is hip core decompression?

Hip core decompression, also known as femoral head decompression, is a surgical procedure performed to treat avascular necrosis (AVN) of the hip. AVN occurs when the blood supply to the femoral head (the ball-shaped top of the thighbone) is compromised, leading to the death of bone tissue. Hip core decompression aims to relieve pressure within the femoral head, reduce pain, and promote the regeneration of healthy bone.

Overview of the hip core decompression procedure:

1. Procedure: Hip core decompression involves making a small incision near the hip joint and creating a channel or tunnel in the femoral head using a surgical drill or other specialized instruments. The purpose of this tunnel is to relieve pressure, decrease the size of the necrotic area, and improve blood flow to the affected bone.

2. Bone Drilling: The surgeon carefully drills a hole or multiple holes into the femoral head. This drilling creates a pathway for new blood vessels to form and supply blood to the necrotic area, promoting healing and regeneration of the bone.

3. Decompression Techniques:
Multiple Small Hole Decompression: In this technique, multiple small-diameter holes are drilled into the femoral head to create a network of channels for improved blood flow.
Core Decompression: A larger diameter hole is drilled into the femoral head, reaching the central area of the necrosis. This technique aims to remove a core of the necrotic bone, relieve pressure, and stimulate new bone growth.

4. Bone Grafting: In some cases, bone grafting may be performed during hip core decompression. A bone graft, typically taken from another part of the patient's body or sourced from a donor, is placed into the drilled hole to support bone regeneration and provide a scaffold for new bone growth.

5. Postoperative Care: After the procedure, patients will need to follow a rehabilitation program, including physical therapy, to restore strength, range of motion, and mobility. Weight-bearing restrictions may be implemented to allow for proper healing of the bone.

Hip core decompression is most effective in the early stages of avascular necrosis when the necrotic area is relatively small. The success of the procedure depends on various factors, such as the extent and location of the necrosis, the overall health of the patient, and adherence to postoperative care and rehabilitation protocols. It's important to consult with an orthopedic surgeon who specializes in hip conditions to determine the most appropriate treatment approach for avascular necrosis.
