How to Test for Knee Ligament Injury: A Comprehensive Guide

Anterior Drawer Test:

Similar to the Lachman test, the anterior drawer test evaluates ACL integrity. The patient’s knee is flexed to 90 degrees, and the tibia is pulled forward. Excessive anterior translation indicates ACL injury.

Pivot Shift Test:

The pivot shift test assesses for ACL tears and rotational instability. With the patient’s knee flexed and internally rotated, a valgus force is applied while extending the knee. A positive test results in the tibia subluxing anteriorly.

Valgus Stress Test:

The valgus stress test evaluates MCL integrity. With the knee slightly flexed, a valgus force is applied to the knee. Pain or laxity along the medial joint line indicates MCL injury.

Varus Stress Test:

The varus stress test assesses LCL integrity. With the knee slightly flexed, a varus force is applied to the knee. Pain or laxity along the lateral joint line indicates LCL injury.
